Monday Morning Makeover - Read Me First

By applying for a "Monday Makeover" with WKBT and Sue Kolve’s Salon & Day Spa, I agree to the following conditions:

• I will available to participate in the makeover process – which occurs at Sue Kolve’s Salon & Day Spa, 230 Main St., Onalaska, on a Monday morning between 8:45 a.m. and 12:45 p.m.
• I will be available for 2 ½ - 3 hours at the salon for taping.
• I am willing to appear live on the WKBT set on a Monday morning. I will commit to being at WKBT from 6 a.m. to 7 a.m.
• I am willing to allow the team at Sue Kolve's to truly make me over, which could include hair cut, hair color, make up and nail services.
• I understand that if I am unable or unwilling to fulfill these expectations, my credit card will be charged for the services I receive at Sue Kolve’s.
• I will sign a legal wavier that stipulates:
  • The undersigned talent, does hereby consent to the use by QueenB Television, LLC, doing business as WKBT, 141 S. 6th Street, La Crosse, Wisconsin 54602-1867, does hereby sell, assign and grant unto station the irrevocable and unconditional power, right, privilege and permission to make, produce, reproduce, exhibit, distribute, publish and transmit by means of live broadcast or cablecast, film, videotape, web or any other similar mechanical or electronic method, the talent’s name, voice, picture, likeness, poses, actions and any combination of any of these in connection with the production, and any exploitation of the production, in whole or in part.
  • Talent agrees to conduct himself/herself in a competent, timely and responsible manner that will reflect favorably on station. Talent understands that the production is subject to station’s sole approval, direction and editorial control at all times. Talent further understands that station is not responsible for any agent’s commission in connection with the making or performance of these agreements and any such commission must be borne and paid for solely by talent.
  • Talent moreover understands that station shall be the sole owner of the copyright in the production involving talent made hereunder and that such production, or any portion thereof, may be used at any time and from time to time hereafter throughout the world.
  • Talent hereby releases and discharges the sponsor and the station, its officers, directors, employees and agents from any and all liability arising out of or in connection with the making, producing, reproducing, exhibiting, distributing, publishing, transmitting any means or otherwise using the above mentioned production.
  • The Undersigned talent hereby certifies and warrants that talent has read document in its entirety and understands all of its terms and provisions.
